You can enable debugging and make sure that the assignment earlier in
your dialplan is always assigning a value.
That is, if you assign nothing to CDR(accountcode), of course the
value will be nothing. Is there any chance that there's occasionially
something going wrong earlier such that there's no value to assign in
the first place? Debugging to a log would confirm that the problem is
between the assignment and the write of the CDR, rather than a problem
with something else before that step.

Another problem might be alternate entry points in the dialplan. For example,
spool files and the manager Originate command can both set up calls at a
context, extension, and priority of their own choosing, rather than going
through all steps of your dialplan (as external calls would). Also, SIP calls
can setup to any extension in your entry context, including non-numeric
extensions. So you can't depend upon that, either.

I am trying to get a small list of bugs closed in regards to CDR's.
So, I advise that you check out
http://svn.digium.com/svn/asterisk/team/murf/CDRfix4 if you are using
1.4
or
http://svn.digium.com/svn/asterisk/team/murf/CDRfix6 if you are using
1.6.0 or trunk,
and see if the problem stays the same, gets worse, or gets better. A
short
response to me directly will help me out a lot.

Assigning CDR(accountcode) is generally the first thing I do in my dialplan
at every entry point. I debug it using Noop() cmds seeded through my
dialplan, and it`s set correctly. At one point, it just gets unassigned...
And I`m not using any spool files or manager commands, just basic dialplan
commands, so that doesn't help, but thanks trying, it could have been the
reason.
