No expert on this, but have you tried to update /usr/local/freeswitch/
conf/lang/en/en.xml to insure you include my-ivr.xml?
So in addition to this:
<X-PRE-PROCESS cmd="include" data="demo/*.xml"/>
have this as well:
<X-PRE-PROCESS cmd="include" data="my-ivr/*.xml"/>
